Personally completed Health Forms: When you book for an appointment you may be asked to complete a Health Form and provide specific personal contact information, background information about yourself, and answer personal health questions. These are either a paper printed form or an digital form that you will complete. Any information you provide will be kept strictly confidential and is kept in a client filing system accessible only by Julia Smith. You are required to honestly and fully disclose information on these forms, so we are able to best treat you.
